  1. The information provided in this application is accurate and complete to the best of my knowledge.
  2. I am duly authorised to make this application on behalf of the school named above.
  3. The school agrees to engage with SparkLab Technologies Limited in good faith to evaluate the Yello Booth service.
  4. Submission of this form does not constitute a binding contract. A formal service agreement will be issued separately.
  5. The school acknowledges that SparkLab may collect and process the information provided in accordance with its Data Protection Policy.
